How to Set Up MyHealthONE Proxy Access
You can Set Up MyHealthONE Proxy Access to create a MyHealthONE account for your loved one or to allow someone else to access your information.
MyHealthOne permits access to a loved one’s health information. Both you and the patient for whom you’re giving care will need to permit the connection of accounts.

Requirements to Set Up MyHealthONE Proxy Access
- Must be at least 18 years old
- Have an active email address
- Present photo identification (for adult patients only)
- Should be a trusted individual
Steps to Set Up MyHealthONE Proxy Access
To create an account for someone other than yourself, or to allow someone else to view the information in your MyHealthONE account, you will need to set up proxy access.
Please select the form below that reflects the patient’s age and follow the instructions on the first page, or, while in the hospital, ask any registration team member for the appropriate form and complete it before leaving the hospital.
How to Set Up MyHealthONE Proxy Access for Patients aged 0-12
- A parent or legal guardian must call the MyHealthONE portal support team at (855) 422-6625
- Access expires on the patient’s 13th birthday
How to Set Up MyHealthONE Proxy Access for Patients aged 13-17
Proxy Express Waiver – English
- Outside Texas, a parent or legal guardian with a minor present must contact the MyHealthONE portal support team at (855) 422-6625
- For patients in Texas, a parent or legal guardian does not need the minor present and may directly request access by contacting the MyHealthONE portal support team at the number above.
- Access expires on the patient’s 18th birthday
How to Set Up MyHealthONE Proxy Access For Adult Patients
Proxy Express Waiver – English
- If the patient is unable to sign the proxy express waiver, the patient’s Power of Attorney may contact the MyHealthONE portal support team at (855) 422-6625. The Power of Attorney will be asked to send the Proxy Express Waiver, Medical POA, and a copy of a state-issued photo identification electronically.
- If the patient is unable to sign the proxy express waiver and no Power of Attorney exists for the patient, then the caregiver or proxy enrollment cannot occur.
To create an account for someone other than yourself, or to grant Proxy access to your MyHealthONE account, contact the MyHealthONE portal support team at (855) 422-6625.
If you have already left the hospital, you may need to return to the hospital to fill out and submit the form; please bring a photo ID. It may take a day or two to process the form at the hospital. After 48 hours, please call the MyHealthOne support team at (855) 422-6625 to complete the caregiver enrollment process.
